Heart Monitor on Shabbos

Question:

I recently had a procedure to correct an irregular heartbeat. The doctor is now putting me on a 14 day monitor to assess whether the heartbeat and other functions are back to normal. The monitor is attached to an adhesive strip on my chest. It constantly transmits data to a smartphone device which sends the data to a monitoring center.
Is it prermitted to go out on Shabbos without an eruv wearing the monitor? Another issue is the smartphone gathering the data, is it better to carry it around כלאחר יד so that the data is constantly being updated, or should it be left in one place which means that whenever I go near the phone it will tell the monitor to transmit the data. Thanks for your advice.
